Les r\u00e9sultats d&rsquo;un premier essai clinique pilote ont d\u00e9montr\u00e9 l&rsquo;innocuit\u00e9 et l&rsquo;observance des patients et ont r\u00e9v\u00e9l\u00e9 des tendances d&rsquo;efficacit\u00e9 chez 53 patients atteints de la maladie d&rsquo;Alzheimer l\u00e9g\u00e8re \u00e0 mod\u00e9r\u00e9e trait\u00e9s avec la th\u00e9rapie PBM (Blivet et al.,\u00a0A randomized, double-blind, and sham-controlled trial of an innovative brain-gut photobiomodulation therapy : safety and patient compliance. J Alzheimer&rsquo;s Dis. 2022, 90 : 2). Cette \u00e9tude a fourni\u00a0des informations pr\u00e9cieuses pour\u00a0la conception de notre essai th\u00e9rapeutique pivot, qui \u00e9value\u00a0actuellement les avantages cognitifs de la th\u00e9rapie par photobiomodulation, dans un \u00e9chantillon plus large de patients atteints de la maladie d&rsquo;Alzheimer.<\/p>\n<p><strong>Objectifs<\/strong><\/p>\n<p>Le crit\u00e8re d&rsquo;\u00e9valuation principal de cet essai clinique pivot est l&rsquo;\u00e9volution de la cognition des patients apr\u00e8s 26 semaines de th\u00e9rapie PBM, mesur\u00e9e par le score ADAS-cog. Les fonctions neuropsychologiques, l&rsquo;autonomie, la r\u00e9ponse clinique globale, la qualit\u00e9 de vie, les marqueurs sanguins et f\u00e9caux ainsi que l&rsquo;int\u00e9r\u00eat m\u00e9dico-\u00e9conomique sont \u00e9tudi\u00e9s comme crit\u00e8res secondaires. La s\u00e9curit\u00e9 du traitement RGn600 est \u00e9galement \u00e9valu\u00e9e.<\/p>\n<p><strong>M\u00e9thodes<\/strong><\/p>\n<p>Le RGn600 est un dispositif m\u00e9dical non invasif, fabriqu\u00e9 par REGEnLIFE, qui se pr\u00e9sente sous la forme d&rsquo;un casque et d&rsquo;une ceinture abdominale combinant la technologie PBM des longueurs d&rsquo;onde rouge \u00e0 proche infrarouge et la stimulation magn\u00e9tique statique.<\/p>\n<p>Un essai clinique pivot multicentrique, en double aveugle, randomis\u00e9, contr\u00f4l\u00e9\u00a0a \u00e9t\u00e9 initi\u00e9 en juillet 2023 au G\u00e9rontopole du CHU de Toulouse. Au total, 108 patients pr\u00e9sentant les crit\u00e8res de diagnostic NIA-AA de la MA seront inclus et randomis\u00e9s en un groupe trait\u00e9 (n=54) et un groupe sham (n=54). Ils ont \u00e9t\u00e9 pr\u00e9sent\u00e9s lors de la 15e Conf\u00e9rence internationale sur les maladies d\u2019Alzheimer et de Parkinson (AD\/PD 2021) du 9 au 14 mars dernier, par le Professeur Jacques Touchon, conseiller scientifique sur cet essai clinique.<\/p>\n<p>La technologie novatrice et non-invasive de REGEnLIFE, qui fait l\u2019objet de cet essai th\u00e9rapeutique, repose sur une technique de photobiomodulation visant \u00e0 cibler le cerveau et l\u2019intestin, gr\u00e2ce \u00e0 un casque et un plastron abdominal. Ce dispositif m\u00e9dical in\u00e9dit en photo-m\u00e9decine, baptis\u00e9 RGn, a pour objectif de stimuler les cellules dans le cerveau et l\u2019intestin et de r\u00e9guler l\u2019inflammation, de mani\u00e8re \u00e0 am\u00e9liorer le fonctionnement cognitif et le comportement. Il cible ainsi l\u2019inflammation sur l\u2019axe cerveau-intestin, qui serait impliqu\u00e9 dans le d\u00e9veloppement de la maladie d\u2019Alzheimer ou d\u2019autres affections neurologiques.<\/p>\n<p><span style=\"color: #69abb4;\"><strong>Design de l\u2019\u00e9tude clinique pilote de REGEnLIFE sur la maladie d\u2019Alzheimer\u00a0:<\/strong><\/span><\/p>\n<p>\u00c9tude pilote \u00e9valuant la s\u00e9curit\u00e9 et l&rsquo;efficacit\u00e9 d&rsquo;un prototype de photobiomodulation, RGn, d\u00e9velopp\u00e9 par la soci\u00e9t\u00e9 REGEnLIFE sur l&rsquo;\u00e9volution des performances cognitives chez les patients atteints de la maladie d&rsquo;Alzheimer l\u00e9g\u00e8re \u00e0 mod\u00e9r\u00e9e.\u00a0<a href=\"https:\/\/clinicaltrials.gov\/ct2\/show\/NCT03672474\">(NCT03672474)<\/a>[\/vc_column_text][vc_single_image image=\u00a0\u00bb4652&Prime; img_size=\u00a0\u00bblarge\u00a0\u00bb alignment=\u00a0\u00bbcenter\u00a0\u00bb css=\u00a0\u00bb\u00a0\u00bb][vc_column_text css=\u00a0\u00bb\u00a0\u00bb]Cet essai a port\u00e9 sur des volontaires adultes, \u00e2g\u00e9s de 55 \u00e0 85 ans, atteints, \u00e0 un stade l\u00e9ger \u00e0 mod\u00e9r\u00e9 de la maladie d\u2019Alzheimer. \u00c9quip\u00e9s d\u2019un casque et d\u2019une ceinture abdominale de photobiomodulation, ces patients ont b\u00e9n\u00e9fici\u00e9 de 40 s\u00e9ances de 25 minutes r\u00e9parties sur deux mois et ont \u00e9t\u00e9 \u00e9valu\u00e9s par un ensemble de tests le long de l\u2019\u00e9tude et jusqu\u2019\u00e0 un mois apr\u00e8s l\u2019arr\u00eat du traitement. Cet essai clinique en double aveugle, randomis\u00e9, monocentrique et contr\u00f4l\u00e9 par placebo, a d\u00e9but\u00e9 en 2018 et s&rsquo;est termin\u00e9 pr\u00e9matur\u00e9ment en 2020 en raison de la pand\u00e9mie de COVID-19. Sur les 64 pr\u00e9vus, 53 patients ont \u00e9t\u00e9 randomis\u00e9s en deux groupes (trait\u00e9s et placebo) et 43 patients ont pu b\u00e9n\u00e9ficier du traitement pendant la dur\u00e9e pr\u00e9vue.<\/p>\n<ul>\n<li>Le groupe trait\u00e9 montre des tendances en faveur d\u2019une am\u00e9lioration cognitive, des fonctions ex\u00e9cutives, de la compr\u00e9hension du langage et de la m\u00e9moire verbale par rapport au groupe placebo<\/li>\n<li>La technologie de REGEnLIFE s\u2019est montr\u00e9e s\u00fbre et bien tol\u00e9r\u00e9e par les patients trait\u00e9s<\/li>\n<li>Ces r\u00e9sultats ouvrent de nouvelles perspectives pour le d\u00e9veloppement de ce dispositif in\u00e9dit pour la pr\u00e9vention et le traitement des affections neurologiques par une stimulation \u00e0 la fois c\u00e9r\u00e9brale et intestinale<\/li>\n<\/ul>\n<p>Le crit\u00e8re principal d\u2019efficacit\u00e9 \u00e9tait mesur\u00e9 par l&rsquo;\u00e9volution du score total de l\u2019ADAS-Cog (Alzheimer\u2019s Disease Assessment Scale) entre l&rsquo;inclusion et les deux mois de traitement. En termes de s\u00e9curit\u00e9 d\u2019usage, le dispositif REGEnLIFE RGn\u00a0s\u2019est av\u00e9r\u00e9 s\u00fbr et aucun effet ind\u00e9sirable grave n&rsquo;a \u00e9t\u00e9 signal\u00e9. L&rsquo;observance des s\u00e9ances de traitement \u00e9tait tr\u00e8s bonne pour une grande majorit\u00e9 des patients 92 %), et a pu \u00e9galement confirmer la bonne tol\u00e9rance du dispositif. Si le crit\u00e8re principal d&rsquo;efficacit\u00e9 n&rsquo;a pas \u00e9t\u00e9 atteint en termes statistiques, une tendance est claire quant \u00e0 l\u2019am\u00e9lioration d\u2019un ensemble de fonctions cognitives. Les r\u00e9sultats de cette \u00e9tude pilote ont montr\u00e9 que la technologie REGEnLIFE est s\u00fbre et bien tol\u00e9r\u00e9e par les patients. Ces r\u00e9sultats tr\u00e8s encourageants de s\u00e9curit\u00e9 et d&rsquo;efficacit\u00e9 doivent \u00eatre confirm\u00e9s dans un essai clinique pivot ou phase III.<\/p>\n<p>\u00ab\u00a0La strat\u00e9gie th\u00e9rapeutique de la maladie d\u2019Alzheimer, pour \u00eatre efficace, devra int\u00e9resser plusieurs cibles. Les traitements m\u00e9dicamenteux visant les deux prot\u00e9ines caract\u00e9ristiques du processus Alzheimer (prot\u00e9ines b\u00eata-amylo\u00efde et tau) doivent \u00eatre compl\u00e9t\u00e9es par d\u2019autres th\u00e9rapeutiques visant des m\u00e9canismes moins sp\u00e9cifiques mais tr\u00e8s importants dans la cascade physiopathologique telle que l\u2019inflammation et le stress oxydant. La technologie de photobiomodulation d\u00e9velopp\u00e9e par REGEnLIFE agit tr\u00e8s en amont sur cette cascade (mitochondrie, inflammation, stress oxydant). Elle pourrait \u00eatre le compl\u00e9ment non-m\u00e9dicamenteux de la strat\u00e9gie th\u00e9rapeutique du futur.
